Age-related macular degeneration symptoms: Early signs and when to see an optometrist

    Recognising age-related macular degeneration symptoms

Age-related macular degeneration (AMD) affects the macula, the central part of the retina used for detailed tasks such as reading and recognising faces. Early detection is important so you can get an assessment and appropriate care. For recommendations on supplements mentioned here, see our For recommendations on supplements mentioned here, see our Vision 20 review.

What are the early symptoms to look out for?

AMD most often develops slowly and painlessly. Early signs may be subtle and easy to miss. Common early symptoms people report include:

    • Blurring of central vision — difficulty reading small print or recognising faces.
    • Distortion of straight lines — they may look wavy or bent (metamorphopsia).
    • Need for brighter light to read — or increased difficulty seeing tasks in dim light.
    • Colours appearing less vivid or washed out.
    • A dark or empty area in the centre of vision (central scotoma) as the condition progresses.

Peripheral (side) vision is usually preserved in AMD, so people may not notice a problem until central vision becomes noticeably affected.

Dry versus wet AMD — how symptoms can differ

There are two main types of AMD. Dry AMD is more common and generally progresses more slowly; it may begin with small yellow deposits called drusen under the retina and gradual blurring. Wet AMD is less common but can cause rapid changes due to abnormal blood vessels leaking or bleeding beneath the macula.

Because wet AMD can lead to sudden and serious vision loss, any rapid change should prompt urgent assessment.

When to see an optometrist or get urgent help

Make an appointment with an optometrist or your GP if you notice any new or worsening central visual changes, in particular:

    • Sudden onset of blurred or distorted central vision.
    • A new dark patch in the centre of your sight.
    • Rapid worsening over days or weeks, rather than months.
    • New flashes, a sudden increase in floaters, or a veil/curtain effect over part of your vision — these may indicate a detached retina and need emergency care.

If you have symptoms suggesting wet AMD (rapid vision change or distortion) you should be seen urgently; eye clinics sometimes offer rapid-access appointments for suspected wet AMD.

What an eye assessment may involve

An optometrist or ophthalmologist will ask about symptoms, general health and family history, then perform tests such as:

    • Visual acuity (reading a chart) to quantify central vision.
    • Amsler grid testing — a simple grid you look at to check for line distortion or missing areas.
    • Ophthalmoscopy or retinal imaging to look for drusen, pigment changes or fluid under the macula.
    • Optical coherence tomography (OCT) — a scan that shows cross-sectional images of the retina and detects fluid or thickening.
    • Fluorescein angiography may be used in some cases to map leaking blood vessels (usually in hospital eye units).

    Treatment options and lifestyle measures

Treatments differ depending on whether AMD is dry or wet. Standard medical treatments for wet AMD used in the NHS include anti-VEGF injections that aim to control abnormal blood vessels; other interventions may be considered in specific situations. For dry AMD, management focuses on monitoring and supportive measures.

Lifestyle measures that clinicians commonly recommend include stopping smoking, controlling cardiovascular risk factors (such as high blood pressure), protecting eyes from excessive sunlight with sunglasses, and maintaining a healthy diet rich in leafy greens, oily fish and a variety of fruit and vegetables.

    About vision supplements

Many supplements are marketed as supporting eye health. For example, the makers of Vision 20 market it as a supplement that combines carotenoids (such as lutein and zeaxanthin) with zinc and other ingredients and claim it helps support near‑ and far‑distance vision, low‑light vision and protection against age-related vision decline. Such products are promoted as supporting eye health, but individuals should be aware that marketing claims vary and supplements are not a substitute for medical treatment.

The Royal College of Ophthalmologists and other UK bodies note specific supplement formulations (AREDS/AREDS2) have been studied in clinical trials for certain groups of people with AMD, but you should not self-prescribe high-dose vitamins without professional advice. If you are considering any supplement, discuss it with your optometrist, ophthalmologist or GP — especially if you take other medicines or have medical conditions.

Tip: Keep an Amsler grid at home. Check each eye separately weekly and report any new waviness or blind spots to your optometrist promptly.

    Who should be especially vigilant?

Be particularly alert if you are aged over 50, have a family history of AMD, are a smoker, have a cardiovascular condition or have had significant long‑term exposure to sunlight. Regular eye examinations are advised for older adults and those at higher risk.

Early recognition and timely assessment by an eye professional are the best ways to preserve vision and access appropriate care.
